The role This isn't another startup building a nice-to-have. This is critical API infrastructure for diagnostic healthcare, connecting wearables, labs, and devices so health companies can deliver personalised, proactive care. They stream time-series data from millions of wearables every second, process millions of lab tests annually, and are targeting 40 million diagnostics next year. All while being profitable, with revenue 4x'd in the last year and their Series A untouched. Backed by leading investors, this is a company with real traction and a profound mission.

You'll join a small, growing engineering team (6 to 18 this year) to own backend services and pipelines that ingest, process, and serve this high-volume medical data. This is a rare seat: genuine 0→1 ownership on systems already running at production scale. It's building reliability into critical infrastructure, not prototyping. You'll iterate with discipline alongside domain experts, not "move fast and break things" in a domain where bad decisions impact real patients. While the centre of gravity is backend, you'll go full-stack into TypeScript and Next.js when the work calls for it.

The work

• Design, build, and own high-volume backend services and data pipelines for critical medical data.
• Engineer fault-tolerant systems and durable workflows using tools like Temporal.
• Ship clean, well-designed APIs that customer engineering teams build on.
• Influence the "what" as much as the "how", taking messy problems from 0→1.
• Debug complex production issues across distributed systems.

What You Bring

• Strong backend engineering experience: designing APIs, distributed systems, queues, and debugging complex production issues.
• Experience at a startup, influencing product direction (not just implementation).
• Ability to translate underspecified problems into clear plans and shipped software.
• Strong product judgment to iterate fast, even within hard constraints and complex edge cases.
• Physically based in GMT or EST timezone, with a genuine, specific motivation for healthcare.
